Collaborating with Veterans Affairs and community services, you will focus on health promotion, disease prevention, and addressing acute, chronic, and long-term health… needs based on established goals and care plans for each Veteran. This position will primarily support patients in Oncology and Hematology, facilitating their health management journey through tailored care.

Responsibilities:
• Assess patient healthcare needs and develop or maintain care plans.
• Respond to changes in patients’ health with timely interventions.
• Coordinate comprehensive care and provide direct patient care.

• Facilitate patient admission, transfer, and discharge processes.
• Serve as a Patient Aligned Care Team (PACT) leader, mentoring peers and students.
• Provide case management for complex patients, focusing on Women Veterans.
• Perform accurate physical assessments and triage patient needs.
• Engage with community resources for coordinated patient care.
Qualifications:
• Bachelor of Science in Nursing (BSN).
• Graduation from an accredited nursing program (NLNAC or CCNE).
• Current RN license in any state or commonwealth.
• Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification.
• U.S. Citizenship is required.
• Successful completion of necessary background checks and VA credentialing policies.
Education Requirements:
• Bachelor of Science in Nursing (BSN) from an accredited institution.
Education Requirements Credential Category:
• Nursing Degree: Bachelor Degree.
Experience Requirements:
• Minimum of 3 years of experience in case management.
• Minimum of 3 years of experience in hematology/oncology.
• Proficiency in EMR/EHR systems and technology.
