Key Responsibilities
• Provide clinical leadership and oversight in the management of patient care, ensuring exceptional delivery and outcomes.
• Lead performance enhancement initiatives targeting Part B metrics, referrals, and specialty program engagement.
• Design and execute cutting-edge value-based care programs, focusing on referral efficiencies, integrated services, and e-consults.
• Spearhead projects centered on dementia care and other critical areas in geriatrics, including the development of clinical pathways and educational resources.
• Collaborate with organizational stakeholders to formulate and implement robust network and specialty care programs.
• Leverage data analytics to drive strategic decisions, engage at the patient level, and devise targeted outreach methods.
• Work closely with educational leaders to create and disseminate impactful training materials for primary care practitioners.
• Provide virtual consultations for complex cases, alongside direct patient care responsibilities.
• Engage with market stakeholders through travel to enhance collaborations and partnerships.

Required Skills
• Board-Certified in Geriatrics and either Internal Medicine or Family Medicine.
• Demonstrated experience in teaching and mentoring healthcare teams.
• Profound understanding of value-based payment structures and clinical program development.
• Expertise in data analysis, business case presentation, and strategic communication.
• Flexibility to accommodate various time zones and work schedules.

Qualifications
• Minimum of 2 years in designing and leading integrated specialty care or dementia care initiatives.
• At least 7 years of experience in direct clinical value-based care or complex primary care settings.
• Proven record of success in collaborative care environments.
