Key Responsibilities
• Evaluate individual needs and devise customized care plans.
• Facilitate connections to vital resources, including social services, community support programs, and financial assistance.
• Advocate for members to ensure their preferences and requirements are prioritized by the healthcare team.
• Maintain clear and continuous communication with all relevant stakeholders to support effective care coordination.
