Job Description
CVS Health is seeking a dedicated and skilled Remote Nurse Consultant to join our dynamic team. Requirements
- Active RN license in the state of residence
- Bachelor’s degree in Nursing (BSN) preferred
- Minimum of 3 years of clinical experience
- Previous experience in case management or consulting roles is a plus
- Proficient in using electronic health records (EHR) systems
- Strong analytical and problem-solving skills
- Excellent verbal and written communication skills
Responsibilities
- Provide remote consultation and clinical guidance to patients and healthcare providers
- Conduct comprehensive health assessments and develop care plans
- Coordinate and manage patient care, ensuring continuity and quality
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Utilize telehealth platforms to deliver services and monitor patient progress
- Educate patients and families on health management and treatment options
- Maintain accurate and up-to-date patient records in compliance with regulatory standards
