Job Description
CVS Health is seeking a dedicated and compassionate Virtual Care Coordinator Nurse to join our dynamic team. This role is crucial in providing high-quality virtual care management to our members, ensuring they receive appropriate and timely healthcare support. As a Virtual Care Coordinator Nurse, you will be the linchpin between our patients and healthcare providers, ensuring seamless care coordination and management in a remote setting. This position offers the flexibility of working from home while making a significant impact on patient health outcomes.
Responsibilities
- Coordinate and manage virtual care for patients by working closely with healthcare providers.
- Conduct telephonic and video consultations, assessments, and follow-ups with patients.
- Develop and implement personalized care plans based on patient needs and health conditions.
- Provide education and support to patients regarding their health conditions, treatments, and medication adherence.
- Monitor patient progress, identify potential health issues, and take proactive measures to address them.
- Document all patient interactions, care plans, and progress notes in the electronic health record (EHR) system.
- Collaborate with other healthcare team members to ensure comprehensive and coordinated care.
Requirements
- Active Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of clinical nursing experience, preferably in a care management or telehealth setting.
- Strong communication and interpersonal skills.
- Proficiency in using electronic health record (EHR) systems.
- Ability to work independently and manage multiple priorities effectively.
- Strong organizational skills and attention to detail.
- Commitment to patient confidentiality and data security.
Educational Qualifications
- Associate’s or Bachelor’s degree in Nursing (ASN or BSN) from an accredited institution.
Experience
- Minimum of 2 years of clinical nursing experience.
- Experience in care management or telehealth is highly preferred.
