Sentara Health is seeking to hire a qualified individual to join our team as Care Coordinator !
This is a Temporary Full-time- remote position for applicants residing in Virginia!
This temporary position is set to last a minimum of 3-6 months.
Standard Working Hours: Monday Friday, 8am 5pm.
Responsibilities :
Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
Education
Must possess a Bachelor’s degree in Health & Human Services or one of the following related Fields: Art Therapy

Child Development
Cognitive Sciences
Community Mental Health Counseling (MH, Vocational, Pastoral, etc.)
Counselor Education
Early Childhood Development
Educational Psychology

Certification/Licensure

Experience
1-year Long Term Care experience required.
Health plan and Medicaid experience preferred.
Strong communication, assessment, and advocacy skills to support patients through various stages of care.
Demonstrated skill in fostering and monitoring multidisciplinary collaboration to support coordinated patient care.
