Brief Description
We are seeking a Care Enablement Coordinator to support Nurse Care Coordinators in providing comprehensive care coordination services to patients. The ideal candidate will have experience in healthcare and possess excellent communication and organizational skills. The Care Integration Navigator will work closely with Nurse Care Coordinators and patients to ensure that patients receive timely and appropriate healthcare services.
Essential Functions
Primary duties include, but are not limited to:
• Work with Nurse Care Coordinators to identify patients who require care coordination services.
• Communicate with patients, families, and healthcare providers to gather information necessary to optimize patients care.
• Monitor patient progress and communicate with Nurse Care Coordinators to adjust care plans as necessary.
• Maintain accurate patient records and documentation of care plans, progress notes, and other relevant information.
• Participate in team meetings and communicate regularly with Nurse Care Coordinators, patients, and healthcare providers.
• Collaborate with healthcare providers and community organizations to identify resources that can support patients in achieving their healthcare goals.
• Maintain strong relationships with healthcare providers, patients, and their families.
• Assist with scheduling appointments, coordinating transportation, and arranging for language interpretation services as needed.
• Develop community resource tools that can be used by Nurse Care Coordinators and patients to identify resources available to support healthcare needs.
• Identify when a patient has a clinical need that requires the attention of a Nurse Care Coordinator and escalate appropriately.
• Work independently in a remote environment and communicate effectively with the care team.
• Maintain up-to-date knowledge of healthcare policies and regulations, as well as changes in the healthcare landscape that could impact patient care.
• Collaborate with other members of the care team to ensure that patients receive coordinated and comprehensive care.
• Identify opportunities for improvement in care coordination processes and contribute to quality improvement initiatives.
• Communicate effectively with patients and their families, providing support and guidance as needed.
• Participate in ongoing professional development activities to enhance skills and knowledge related to care coordination and healthcare.
• Adhere to ethical and professional standards of conduct, including maintaining confidentiality of patient information and respecting patient autonomy.

Qualifications
• The ideal candidate for the Care Enablement Coordinator position should possess at least 2-3 years of experience in healthcare, preferably in care coordination or related roles.
• Excellent communication and interpersonal skills are a must, as the Care Enablement Coordinator will be interacting with patients, families, and healthcare providers regularly.
• The ability to work independently in a remote environment is also essential for success in this role.
• Candidates should have strong organizational skills and attention to detail, as well as the ability to develop and maintain accurate patient records.
• Additionally, candidates should possess knowledge of healthcare policies and regulations, as well as an understanding of the healthcare landscape and available resources for patients.
• In addition to the above qualifications, candidates with experience in developing community resource tools and working with underserved populations will be given preference.
• Candidates should also possess strong problem-solving skills, the ability to work in a fast-paced environment, and a commitment to improving patient outcomes.

Knowledge And Experience
• Bachelor’s Preferred but will accept candidates who possess relevant work experience that which aligns with the responsibilities and requirements of the position.
• Experience in care coordination, healthcare, or related roles.
• Familiarity with community resources available to support healthcare needs.
• PC literate, including Microsoft Office products, Outlook, Excel, Word, Adobe, Zoom, MS Teams, and other EMR software.
• Ability to work in a remote environment, as well as work in multiple EMR systems simultaneously.
• Ability to develop and maintain accurate patient records.
• Ability to take initiative and use independent judgment, with a strong understanding of skilled necessity and functional change.
• Commitment to providing patient-centered care and maintaining strong relationships with healthcare providers, patients, and their families.
