What to Expect (Job Responsibilities):
– Conduct outbound calls to newly enrolled VA Medicaid members to establish first contact and complete health screenings
– Assist members in navigating and maximizing their health plan benefits, including scheduling appointments and arranging transportation
– Provide outreach to locate and support disconnected members with special needs
– Conduct research to contact members that Molina Healthcare has been unable to reach
– Participate in ongoing or project-based activities requiring extensive member outreach
What is Required (Qualifications):
– High School Diploma or GED
– Minimum 1 year of experience working with underserved or special needs populations
– Must have a valid driver’s license with a good driving record and reliable transportation
– Experience in a healthcare-related field is a plus
– Ability to collaborate effectively with a multidisciplinary team
How to Stand Out (Preferred Qualifications):
– Associate’s Degree in a healthcare-related field (e.g., nutrition, counseling, social work)
– Bilingual skills based on community needs
– Familiarity with healthcare systems and community resources
– Knowledge of health education and social services
– Current Community Health Worker (CHW) Certification preferred
