Job Overview

Key Responsibilities
• Conduct thorough assessments to identify clinical and psychosocial needs of individuals.
• Address health and safety concerns, including social determinants of health.
• Collaborate with a multidisciplinary team to create a personalized Life Plan.
• Regularly monitor and update Life Plans to ensure optimal care.
• Facilitate access to medical and community resources to promote health and wellness.
• Coordinate referrals for clinical, long-term, and social support services.
• Engage families and natural support systems to enhance care coordination.
• Ensure that services provided are culturally and linguistically appropriate.
• Advocate for individuals to foster self-advocacy and independence.
• Utilize Health Information Technology for efficient documentation and communication.
• Maintain compliance with HIPAA, Medicaid, and all relevant regulatory standards.
• Keep precise and detailed records of services rendered.
• Participate in ongoing training and professional development to enhance skills.
Required Skills
• Proven experience working with individuals with intellectual and developmental disabilities.
• Strong organizational and time management capabilities.
• Ability to operate independently and effectively manage crisis situations.
• Basic proficiency in technology for documentation and communication.
• Valid driver’s license.
Qualifications
• A Bachelor’s degree coupled with a minimum of two years of relevant experience, OR
• A Registered Nurse (RN) license with at least two years of pertinent experience (not restricted to case management/service coordination), OR
• A Master’s degree with one year of relevant experience.
