Job Description
CVS Health is seeking a dedicated and compassionate Remote Clinical Support Nurse to join our team. As a Remote Clinical Support Nurse, you will play a vital role in providing clinical support and guidance to our patients and healthcare providers. You will be responsible for assessing patient needs, coordinating care, and ensuring that our patients receive high-quality, efficient care. This position is fully remote, allowing you to work from the comfort of your own home while making a difference in patients’ lives across the country.
Responsibilities
- Provide telephonic nursing support and care coordination to patients and healthcare providers.
- Perform comprehensive assessments and develop patient care plans.
- Educate patients and their families about health conditions, treatment plans, and medications.
- Coordinate care with healthcare providers, pharmacies, and other healthcare professionals.
- Monitor patient progress and adjust care plans as necessary.
- Document all patient interactions and care plans thoroughly and accurately in the electronic health record (EHR) system.
- Participate in ongoing training and professional development opportunities.
Requirements
- Active and unrestricted Registered Nurse (RN) license in the state of residence.
- Minimum of 2 years of clinical nursing experience.
- Experience in telehealth or remote nursing is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficiency in using electronic health records (EHR) systems.
- Reliable internet connection and a suitable home office setup.
Educational Qualifications
- Associate’s or Bachelor’s degree in Nursing from an accredited program.
Experience
- Minimum of 2 years of clinical nursing experience.
- Experience in telehealth or remote nursing is preferred but not required.
