We are seeking a dedicated Licensed Telehealth Therapist specializing in adolescent and adult care to join our innovative team. Key Responsibilities
• Assess and diagnose a range of behavioral health conditions, both chronic and acute.
• Collaborate with a multidisciplinary health team to develop personalized treatment plans tailored to individual member needs.
• Conduct initial intakes and deliver therapeutic interventions through individual, family, and group sessions.
• Facilitate access to community resources for comprehensive behavioral health support.
• Engage with primary care providers and specialists to monitor treatment progress and discuss care strategies.
• Provide psychoeducation to members, focusing on mental health promotion and preventive care.
• Nurture trusting relationships with patients, aligning care with their goals and values.
• Participate in multidisciplinary case conferences to ensure effective and holistic care.
• Document care and coordination efforts promptly in our electronic medical record (EMR) system.
Required Skills
• 2+ years of outpatient experience with adolescents and adults enrolled in Medicaid, Medicare, or dual-eligible.
• 3+ years of experience in assessing, planning, and managing behavioral health care through direct clinical experience.
• 1+ year of telemedicine experience.
• Strong understanding of trauma-informed and evidence-based psychotherapy.
• Proficiency in electronic medical record documentation and a commitment to leveraging technology for improved care.
• Exceptional judgment in evaluating situations and making informed decisions.
• Effective collaboration skills with diverse healthcare professionals.
Qualifications
• Active LCSW, LMFT, or LCPC license is required.
• Candidates with MFT and CPC internships are also encouraged to apply.
