Job Description
CVS Health is seeking a dedicated and compassionate Telehealth Wellness Nurse to join our dynamic healthcare team. This full-time remote position allows you to provide high-quality care and support to our patients from the comfort of your home. You will play a critical role in managing patients’ health while ensuring they receive the best possible care through telehealth services.
As a Telehealth Wellness Nurse, you will be responsible for offering comprehensive care management and education to our diverse patient population. Your primary focus will be on chronic disease management, preventive care, and patient education. This role requires excellent communication skills, clinical expertise, and a passion for patient advocacy.
Requirements
1. Active and unrestricted RN license.
2. Bachelors degree in Nursing (BSN) preferred.
3. Minimum of 3 years of clinical nursing experience.
4. Previous telehealth experience is a plus.
5. Proficient in using telehealth platforms and electronic health records (EHR).
6. Strong organizational and multitasking skills.
7. Excellent communication and interpersonal skills.
Responsibilities
1. Conduct telehealth consultations with patients to assess their health needs and provide appropriate medical advice.
2. Develop and implement individualized care plans for patients with chronic conditions.
3. Educate patients on disease prevention, healthy lifestyle choices, and self-management techniques.
4. Collaborate with physicians, specialists, and other healthcare providers to ensure comprehensive patient care.
5. Monitor patient progress and adjust care plans as necessary.
6. Document patient interactions and maintain accurate medical records.
7. Participate in continuous education and training to stay updated with the latest in telehealth practices and procedures.

Educational Qualifications
