Requirements
- Active and unrestricted RN license in your state of residence
- Minimum of 2 years of nursing experience, preferably in a clinical setting
- Experience in case management or care coordination is a plus
- Strong communication and interpersonal skills
- Proficiency in using electronic medical records and telehealth platforms
- Ability to work independently and as part of a remote team
- Reliable internet connection and a quiet, private workspace
Responsibilities
- Conduct telephonic assessments to evaluate patients’ health status and needs
- Develop and implement personalized care plans based on patients’ health goals
- Provide education and resources to patients and caregivers on managing chronic conditions
- Coordinate with healthcare providers to ensure continuity of care
- Monitor patients’ progress and adjust care plans as needed
- Maintain accurate and timely documentation in electronic medical records

Educational Qualifications
- Associate’s or Bachelor’s Degree in Nursing from an accredited institution
